UCI Attestation Application Form
This is to be completed by J19, U23 and Elite riders who are competing in events listed on the UCI Calendar (this includes the Australian Road National Championships and submitted directly through the UCI Equipment portal. (link is in the UCI Equipment and Height regulations document)

Rider Application
This is only to be completed by any U15, U17 and Masters riders who are competing in any Time Trials at State or National level. Any riders in these categories who intend competing in J19 or Elite events on the UCI calendar must submit the UCI Attestation Application Form.
